By completing the FORM below, your name will be added to our alumni database. If you think your name is already part of the database, but you have moved recently or have not been in contact with the SSVC Alumni Association recently, we highly recommend that you submit your most recent contact information to our database using the form below.

To submit this information, you must fill in, at a minimum, the REQUIRED information which has an asterisk (*) next to it; however, we prefer that you fill in the complete form.

Upon completing the form, press the SUBMIT button at the bottom of the form and you will be added to the SSVC alumni database. Our database manager will check this address against your address for the SSVC Alumni mailing list. If the address you submitted is different from the one on the SSVC alumni mailing list, our database manager will update the list.
